Adult, Child & Family Stability (ACFS) offers a broad range of programs for adults and families. A detailed list is on the left side of this page. Federal and state programs are designed to provide health care, nutrition support, cash assistance and employment support to eligible consumers. ACFS is a voter registration agency.
How Do I Apply?
ON-LINE APPLICATION AND CHANGE REPORTING
The online application covers all public assistance and Medicaid programs.
Once the on-line application is received, applicants will be contacted by ACFS to schedule an interview. Phone interviews are preferable, but a face to face interview is available upon request.
If it is discovered that you are applying only for Medicaid the interview requirement may be waived.

There are several different application processes based on the program(s) for which you want to apply.
(1) Food Assistance, Ohio Works First (family cash assistance), Disability Financial Assistance (potential income for adults with SSI or SSD pending) Medicaid for the elderly or disabled and Home and Community Based Waiver Services: JFS 07200 application - An interview is required. Once you file a new application, ACFS will provide you with information regarding the phone interview process. A face to face interview is available only upon request. It is preferable that you complete your interview requirement within 5 days of your application file date; this will ensure that you have enough time to provide the required verifications before the 30 day application period expires.
(2) Child and Family Medicaid: JFS 07216 - An interview is not required. You may complete the application and attach your verifications. You will be contacted via mail for any additional documents needed to make an eligibility determination.
(3) Medicare Premium Payment Programs: JFS 07203 – An interview is not required. You may complete the application and attach your verifications. You will be contacted via mail for an additional documents needed to make an eligibility determination.

The Income limits for state and federal programs change every year and we encourage anyone interested to submit an application.
